While aging presents unique changes and challenges, it can also be a fulfilling and dignified season of life. Heartland of Story County offers a number of programs, opportunities, and services to those who are 60+ to enrich their lives and support their wellbeing.

Adult Day Center

Our Adult Day Center offers caregivers respite and those who are 60+ vital caregiving. From life-enriching activities and companionship to medical assistance and nutritious meals, this one-of-a-kind service in Story County brings peace of mind to families. Discover daily high-quality care for the person in your life who needs extra support.

Meals on Wheels
of Story County

In partnership with Mary Greeley Medical Center, Heartland of Story County provides a meal delivery service to homebound individuals. Whether your need is temporary during an illness or hospital stay, or longer-term when you’re unable to leave home and want to continue living independently, we are dedicated to serving daily, nutritious hot meals to your door. 

Outreach Support

If you have a question about information or resources for those who are 60+, we at Heartland of Story County have the connection you need. Serving individual needs on a case-by-case basis, our outreach specialist serves as your bridge to vital services and information for quality aging. 

Congregate Meals Program

Enjoy a nutritious meal and friendly conversation at our weekly Congregate Meals Program for adults 60+. Designed to support both physical health and social well-being, this program offers balanced meals in a warm, welcoming group setting.

Meals are provided by Mary Greeley Medical Center and served every Thursday from 11:30 a.m. to 12:30 p.m. at the Sixty Forward Center.

Reservations are required by Wednesday at 8:30 a.m. To reserve your spot, contact Anne Nordman at (515) 231-1092 or email anordman@heartlandofstorycounty.org.

Voluntary donations are accepted and appreciated. Come for the meal and stay for the friendships.

Community Connections

Navigating the challenges of aging can feel overwhelming — but you don’t have to do it alone. Community Connections offers one-on-one outreach support for adults 60+ and their caregivers throughout Story County.

Led by Anne Nordman, Outreach Specialist, these welcoming sessions provide an opportunity to ask questions, explore available programs, and learn about community resources that can make life a little easier.
