Aging brings with it unique challenges and many uncertainties. Heartland of Story County’s outreach specialist can help. We are dedicated to connecting those 60+ in Story County with community resources to answer their needs. Available for home visits, phone calls, and emails, we meet you where you are to lend a helping hand.
Heartland Outreach Services is an excellent resource for those 60+ or their families who have questions or concerns regarding aging issues and needs. We meet with you, listen to your unique circumstances, and help foster relationships that will bring a higher quality of life to those we serve.
